Why "healthcare marketing agency" in India is a map query, not a search query

The top three results are a local pack. Ten agencies split roughly 1,900 visits a month. What that means if you are not on the right street.

If you sell healthcare marketing in India, the first thing worth knowing is that your buyers' most obvious search term does not behave like a search term at all.

What the SERP actually looks like

Query "healthcare digital marketing agency" from an Indian IP and the first three results are a local pack — a map, three business listings, and a directions link. Organic results start below the fold. That is Google classifying the query as a local services intent, which means proximity to the searcher is doing most of the ranking work.

The size of the prize

We pulled traffic for the full organic top ten. Between them they take roughly 1,900 visits a month. The best-performing page belongs to a DR 6 domain and takes 752 of those. Positions nine and ten take 31 and 56 respectively. For comparison, a single page on a DR 11 American domain takes 1,605 on the equivalent US query.

Why this matters strategically

You cannot out-optimise a competitor's street address. Every technique that works in a content-decided SERP — entity graphs, authored clinical content, credential markup — is neutralised when the ranking factor is where the searcher is standing. Agencies pouring content budget at this term are buying a lottery ticket in someone else's postcode.

Where the demand actually is

The AI-search cluster in India — generative engine optimization, answer engine optimization, LLM SEO, AI SEO agency — carries about 4,500 monthly searches against the healthcare-agency cluster's 1,750. Critically, none of those queries return a local pack. They are national, content-decided, and one of the current page-one results has zero referring domains. That is where a new entrant should be fighting.
