AI SEO services, and how to tell the real ones apart.
The term covers two different things, only one of which is worth buying. Here is the distinction, the three questions that filter a provider in one meeting, and what we will not sell you.
Four questions.
What are AI SEO services?
In honest use, it means two separate things that get sold under one name: making a site visible inside AI-generated answers, and using AI tools to produce search content faster. The first is worth paying for. The second is a production method, not a service.
What should AI SEO cost?
It depends on how much of the technical foundation already exists. Most of the cost in a clinical account is in the first two stages — making the site readable and making the consultants verifiable — and those are one-off rebuilds rather than a monthly retainer.
How do I tell a real AI SEO provider from a repackaged one?
Ask for a Physician schema example they have built that validates. Ask which answer engines they have checked and to see a citation report rather than a rankings report. Ask them to show you the results page for one of your own searches and say whether it is map-decided or content-decided. Very few can do all three.
Does it work for a small hospital?
On the evidence, yes. On the India results page for “answer engine optimization”, the page at organic position two has a Domain Rating of 14 and one referring domain. Authority is not the barrier in this category.
Ask these. Most providers fail at least two.
- “Show me a Physician schema example you have built that validates.” This is structured data describing a clinician — registration, specialty, affiliation — in a form a machine reads. It either validates or it does not. Almost nobody in this market can produce one.
- “Which answer engines have you checked, and can I see a citation report?” Not a rankings report. A record of which systems named the client, on which queries, on which dates.
- “Open the results page for one of my searches and tell me whether it is map-decided or content-decided.” If they cannot do this live, in the meeting, they have not done the work.
We publish these because they filter us too. A buyer who asks all three will get a straight answer from us and will not get one from most of the market, and that asymmetry is the entire reason to publish them.
We looked at every agency ranking for AI search work in India and checked their specialisations. Not one of them works in healthcare. They are generalist shops that have added a service page.
Three things we decline.
- Volume content produced by a model and published unreviewed. On a health site this is a liability rather than an asset, and it is the fastest way to lose the credibility the rest of the work builds.
- A guarantee that a model will name you. It cannot be guaranteed by anybody. We say so on our own LLM page.
- A retainer when the honest answer is a rebuild. If what your site needs is a one-off technical repair and six pages, we would rather do that and stop than bill monthly for maintenance you do not need.
Scope. This page is marketing and search analysis written by a non-clinician. It contains no medical advice and no clinical guidance. Decisions about tests, treatment and care belong with a registered medical practitioner. Search figures are from Ahrefs, India locale, pulled 21 October 2026; volumes are twelve-month averages carrying normal estimation error.
Ask us the three questions. We will answer them in the first call.
And if the honest answer is that you do not need this yet, we will tell you that instead.
